LBNL ICAO recently hosted a successful session of the Department of Energy’s EIP-400 Accident Investigation Course, which concluded with overwhelmingly positive attendee reviews. More than 20 LBNL personnel from across the research and operations div
isions participated in the comprehensive 40-hour training program. The week-long learning experience was divided between classroom instruction and small breakout groups, allowing attendees to practically apply their new skills to a detailed case study.
The EIP-400 course provides the essential techniques and methodologies required to determine the individual, organizational, and systemic causes of workplace events. Participants learned how to scope and plan investigations, gather and control evidence, and utilize core analytical techniques including barrier analysis, change analysis, and root cause analysis. This training equips the workforce to effectively analyze both large-scale incidents and low-grade, non-reportable events to identify leading indicators and accident precursors.
By completing this rigorous program, the participating personnel have satisfied the training requirements of DOE Order 225.1B for DOE Accident Investigators. Bringing this expertise directly to our staff strengthens LBNL’s proactive safety culture, helping us prevent future incidents, reduce their severity, and ensure continuous improvement as a dedicated learning organization.
